PROBLEM STATEMENT
– There is lack of awareness about the Emergency Medical Service (EMS) System
– The general public does not possess basic first aid skills
– There is no standardized toll free national access number to call for emergency medical help
– Adequate number of First Responders/Ambulances are not there
– There are no standardized protocols & medical directives for EMS• In Transit to a definitive care health facility
– Non availability availability of appropriate appropriate & safe transport transport for the injured injured patient in the form of road ambulances, air ambulances etc.,
– The real concept of an Ambulance is missing in India. Existing

consideration to the overall ambulance design( patient care, comfort & ergonomics.
– Currently there is no ‘National Ambulance Code’ in the country
which specifies the minimum National Specifications for various
types of Ambulances, lances, viz ALS, BLS, First Response responder etc.
– The ambulances are inappropriately/inadequately equipped
– In‐adequate care during transportation due to lack of trained

– Sub‐optimal quality care at the existing health facilities due to inadequately skilled manpower
– Lack of SOP’s regarding the handling of a patient on his arrival at the healthcare facility
– Lack of accountability and monitoring mechanisms to ensure timely and optimal care
Miscellaneous
Laws like Motor Vehicles Act 1988 and CMVR 1989 which have been in vogue since 1980 s’ and have not been updated updated since then
– This is lack of coordination between agencies resulting in a mismatch between the existing resources
– There is no appropriate database for the evaluation and enhancement of the services being rendered
– Post Trauma Rehabilitation Facilities for the injured are deficient
– Research into post crash response and emergency care is not there
– There is lack of awareness regarding Hon’ble Supreme Court of
India’s directives regarding the Right to Emergency Care for RTA
victims & the protection available to good Samaritans who offer help to a RTA victim.
– There is no provision to ensure adequate compensation to an RTA
victims in case the accident causing vehicle doesn't have a third
party insurance
– Majority of the drivers do not have a personal mediclaim policy to
cater to their emergency medical needs in case of an accident

EVERY DISASTER IS NOT MAN MADE
It has become a habit now to blame somebody after every natural disaster. Whether it is the earth quake or a tsunami or a tornado or a cloud burst or drought. No doubt man has exploited nature beyond description and has made this a 'sick planet' , but natural disasters make no prior announcements and man is too small before nature's fury. See the history without any worry. ALL BEFORE MAN BECOMING TOO METALIC AND MATERIALISTIC.
Earthquakes and tsunamis
Oct. 5, 1948 - More than 110,000 were killed when a 7.3 quake rolled through the area around Ashgebat in Turkmenistan.
May 22, 1927. A magnitude 7.9 quake near Xining, China, killed 200,000
Sept. 1, 1923. A third of Tokyo and most of Yokohama were levelled when a magnitude 8.3 earthquake shook Japan. About 143,000 were killed as fires ravaged much of Tokyo.
Dec. 16, 1920. China was also the site for the world's third-deadliest quake of the 20th century. An estimated 200,000 died when a magnitude 8.6 temblor hit Gansu, triggering massive landslides.
Dec. 28, 1908. Southern Italy was ravaged by a 7.2 magnitude quake that triggered a tsunami that hit the Messina-Reggio-Calabria area, killing 123,000.
Volcanic eruptions
May 8, 1902. Mt. Pelee erupted on the Caribbean island of Martinique, destroying the capital city of St. Pierre. Up to 40,000 were killed. The day before, a volcano had killed 1,600 people on the nearby island of St. Vincent and five months later Mt. Santa Maria erupted in Guatemala, killing another 6,000.
(Two of the most famous eruptions took place before 1900. In 1883, two-thirds of the Indonesian island of Krakatoa was destroyed when a volcano erupted. A resulting series of tsunamis killed more than 36,000. In 79 CE, Mt. Vesuvius erupted in southern Italy, destroying the ancient Roman city of Pompeii and two other communities. Thousands died.)
Hurricanes, cyclones and floods
Aug. 5, 1975. At least 85,000 were killed along the Yangtze River in China when more than 60 dams failed following a series of storms, causing widespread flooding and famine. This disaster was kept secret by the Chinese government for 20 years.
August 1971. An estimated 100,000 died when heavy rains led to severe flooding around Hanoi in what was then North Vietnam.
Nov. 13, 1970. The Bhola cyclone in the Ganges delta killed an estimated 500,000 in Bangladesh. Some put the complete death toll as high as one million.
June, 1938. Nationalist Chinese soldiers, under the direction of Chiang Kai-Shek, blew up dikes around the Yellow River to stop Japanese troops from advancing. More than half a million people died in the resulting flood.
May-August 1931. Massive flooding of China's Yellow and Yangtze rivers led to almost four million deaths from drowning, disease and starvation. The flooding of the Yangtze also killed an estimated 100,000 in 1911 and 140,000 in 1935.
Hurricane Galveston - Sept. 8, 1900
Galveston was known at the end of the 19th Century as the "Jewel of Texas" until the single deadliest natural disaster in U.S. history wiped away much of what had been a booming future. The bustling island community had been the hub of the cotton trade and Texas' largest city. Progress bred complacency though, which became apparent when city officials and residents decided against building a seawall to protect the city. When the category 4 hurricanewith estimated 135 mph winds made landfall in the early morning, buildings crumbled under the force of 15-foot-high waves. By late afternoon, the entire island was submerged. An estimated 8,000 people perished. Although the city was successfully rebuilt, it never regained the prosperity that earned it a reputation as the "New York of the south.
Dust Bowl - Early 1930's
Prior to the early 1930's, the Great Plains was a farmer's paradise. Rising demands for wheat spurred settlers to plow much of the southern plains' grassy soil to meet this need. The land was eventually exposed to erosion, since grass and tree roots that had held the moist soil in place during dry times were replaced by cash crops. A decade-long drought transformed the loose topsoil into dust, which windstorms swept up and blew eastward, darkening skies as far away as the Atlantic Coast. With most of the areaĆ¢€™s crops decimated, a third of the farmers turned to government aid, while around half a million Americans were left homeless.
keechobee Hurricane - September 16, 1928
When the evacuated residents of Lake Okeechobee learned that a hurricane hadn't arrived on schedule, many returned home thinking that they had been spared. The storm, however, slammed ashore later on the evening of September 16th with sustained 140 mph winds. Such intensity broke a small dike at the lake's south end, resulting in weeks of heavy flooding that claimed at least 2,500 lives.

Campaigns to reduce choking pollution levels should not choke the poor. At this rate one wonders people may demand that humans should not exhale as 'The exhaled air isn’t just carbon dioxide; it contains a mixture of other gases. Human breath contains volatile organic compounds(VOCs). These compounds consist of methanol, isoprene, acetone, ethanol and other alcohols. The exhaled mixture also contains ketones, water and other hydrocarbons.' So should we stop breathing out? There is a limit on both sides. Humans have to survive on a living earth. That's all. For that we have to take every step that we can. It is humanly impossible to supply cooking stoves that don't harm the earth in the near future. Yes, the solar energy is in abundance. But it would take decades or even a century for the technology to reach the poorer sections. Let the campaigns be realistic and simplistic and not cosmetic.

O.K. It is agreed that one should not smoke in public places. At home? The kids will suffer... out side... the neighbours will suffer as we don't live in separate bungalows... then the only option is to smoke in places assigned for the purpose. But the BEST OPTION is to SMOKE THE HABIT OF SMOKING ITSELF.

Centuries back William Wordsworth lamented that man has disassociated himself from Nature and the other fellow creatures and is hence responsible for his miserable plight. The poet is not to be seen as an admirer of Nature but a worshipper who believes that Nature has a soul that is living and there is a link between the soul of Nature and that of all other living beings. But, unfortunately man has failed to see the link and has been
indulging in misdeeds that affect this vital link.

Hence care for this Earth is in the air for quite sometime. But the 'machine led man' is misled and has made this planet a 'material dust bowl'. Greed is seen in his every deed and there is no end indeed. Let every one of us put just one step ahead and let it be for caring this Nature. Let the effort may be small but it should be by all.

Is nature cruel?
Man's nature is to go against 'nature'. Many a time he calls it as 'adventure'. He would like to fly like a bird and swim like a fish. How long and how far this 'un naturalism' would go , none knows. The cloud bursts and the resultant flash floods are an annual ritual now for Uttarakhand and seen in the adjacent states like HP and J &K state. Blame games have begun and in these blind games none is going to win but all will lose. When millions of cusecs of waters come gushing down on sloppy areas at an altitude of 3000m and above, man's capabilities to with stand get limited.
Some lessons from this tragedy.
* Gods are everywhere and hide not your urge to explore and tour distant and high lands in the name of spiritualism
* Development should not be seen in cement and steel but the quality of life you lead (Bhutan king measures it in happiness index)
The annual monsoon rains sustain India's agriculture but also cause flooding that routinely claims lives and damages property.
Presence of an effective communication dissemination system would have lessened the magnitude of the disaster caused in Uttarakhand. The Meteorological Centre, Dehradun had alerted various departments of the state government about the possibility of heavy rains and landslides in the state, 48 hours prior to the intervening night of June 16 and 17 that brought widespread damage. However, it now emerges the alert failed to percolate down to the masses at large, worsening the magnitude of the damage, especially in the Char Dham Yatra route.
Uttarakhand faced the worst hit of rains from the night of June 16 and major damages were caused on June 17 in the hills. The Met Center had issued alerts to various governmental departments two days in advance. Sharma said that establishing an all-weather communication system was the need of the hour. Currently, only three community radios are reportedly functional in Uttarakhand.
"Setting up of a large number of community FM radios can avert such a disaster in future. Also, distribution of low-cost and low-maintenance technologies like tone alert hand-winding radios especially in the rural areas, will ensure that weather alerts reach to the public in time, averting such disasters," he concluded.
